Travel Risk Assessment

If you are travelling abroad please make sure you contact us at least 4 weeks before travel and preferably longer, so that we can assess you and arrange any vaccinations that may be necessary. If you contact us late, we may not be able to vaccinate you. To help the Travel Nurses assess your travel needs it is important that they are in receipt of the assessment form before your appointment.
